Right to Request Restrictions: You have the right to request a restriction or limitation on the use or disclosure of your PHI for treatment, payment, or healthcare operations.

Right to Request Confidential Communications: You have the right to request that we communicate with you about medical matters in a certain way or at a certain location.

Right to a Paper Copy of This Notice: You have the right to obtain a paper copy of this notice upon request.

Changes to This Notice

We reserve the right to change this notice at any time. The new notice will be effective for all PHI that we maintain at that time. We will post a copy of the revised notice on our website and provide a copy upon request.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of Health and Human Services. To file a complaint with us, contact:

Definitions

  • "EMT": Emergency Medical Technician, a certified healthcare provider trained to provide emergency medical care at the basic life support level.

  • "PHI": Protected Health Information, any information about health status, provision of healthcare, or payment for healthcare that can be linked to an individual.

  • "Services": The emergency medical services provided by EMT ELITE, including on-site emergency care, event medical services, training programs, and other related services.

  • "User": Any individual who accesses or uses our website or services.

  • "Terms": These Terms of Service, which govern the use of our website and services.

  • "Contingency": A percentage added to the base cost to cover unforeseen expenses related to the provision of services.

  • "Retainer Fee": A fee paid upfront to secure our services for a specified period, which may be applied towards the total cost of services provided.

  • "HIPAA": Health Insurance Portability and Accountability Act, a U.S. law designed to provide privacy standards to protect patients' medical records and other health information.

  • "Non-Discrimination Statement": A declaration that EMT ELITE does not discriminate on the basis of race, color, national origin, age, disability, or sex in its services.

  • "Intellectual Property": All content, features, and functionality on the website, including but not limited to text, graphics, logos, icons, images, audio clips, video clips, data compilations, and software.
